Membership Transfer:
You may transfer your membership to a family member. In the event of a transfer the "new member" will be the responsible party and billing information will be required as well as a signature. There is a $99 transfer fee. New Member must be 18 years old.

 

IFPA Member Access and Affiliates:

Your QLS membership will also include access to over 1,200 fitness and sports clubs worldwide. Please contact one of our customer service representatives for a club destination nearest you when you travel.

Youth Policies and Procedures:
• No children under the age of 10 allowed on the workout floor. No exceptions.
• Children 10-12 years old must be within 3 feet of a parent or guardian at all times while on workout floor.
• ACSM guidelines strongly recommends that children ages 10-12 years old do NOT exercise on strength equipment.
• All youth ages 10-15 are required to go through the QLS Youth Start Program with a parent before they are allowed to exercise. Youth ages 16-17 must go through QLS Youth Start Program (parental supervision not required) before they are allowed on workout floor.
• Proper footwear is required. No open-toed shoes or bare feet allowed.
• Closed-toe footwear is required for rock-wall climbing.

 

POOL: WE DO NOT HAVE LIFEGUARDS ON DUTY!

• No one under the age of 13 is allowed in the pool area unsupervised.

• How do I cancel my membership?

-  Click here to print down form: Cancellation Request.pdf  Please note on the top of this form what qualifies you to cancel your membership before it expires. Cancellation form and your QLS Key Tags must be sent in via certified mail 30 days prior to a move with proof of relocation. 

• How do I freeze my QLS membership? - This is a courtesy that we extend to our members.  If you are on a term membership you are responsible for the complete term as defined on your membership agreement. You may freeze your membership up to 3 months per calendar year due to several reasons. It is important to note that the length of your freeze is added to the end of your agreement expiration date. If your freeze is due to medical reasons, simply have your physician send us or give you a signed letter noting your medical condition and the length needed for your membership freeze. Note: If we receive your freeze request by the 5th of the month, your membership freeze will be in effect at the end of the month. If we receive your freeze request after the 5th of the month, you will be billed for the following month.

• How do I change my billing information? - To change any billing information, simply stop by the QLS front desk and fill out a “Change of Contract Agreement” form. Only a QLS member can make these changes. You must provide a voided check or credit card information at the time the change form is completed. Any changes to billing must be made a full 30 days prior to your next billing date.

• How do I change my address or telephone information on my membership? - To change this information, stop by the QLS front desk and fill out a “Change of Contract Agreement” form. Only QLS members can make these changes.
